One of my forms is showing up as disabled

    I created a JotForm contact form a few years ago ... and just learned that it is no longer working. Can you please tell me how to fix the issue? Do I need to create a brand new contract form?

    It looks like the form you have linked above belongs to account pagesage, which has been automatically suspended by our system.

    I have checked in with our reviewers team in regards to reactivating the account and, thus, the form, and will update you via this thread as soon as possible.

    The pagesage account was mistakenly suspended by our system and has now been reactivated: the forms should also reactivate as well.
