Common form element used for Yes or No option is the Single Choice Form Element

I noticed that your form has use the Single Option form element

You can hide/show fields base on the user input Yes/No option. I.e. Set the below field as Hidden field.

Add Conditions to show/hide the field base on the Yes/No input

Make and adjustment to the existing conditions so that choosing No will not hide the Yes/No question field.
