Can I create agreements for people to sign in a pdf instead of answering questions via a form?

I want to add an arbitration agreement to my online forms and am trying to figure out if I can do it. I've added a pdf of the form below I want filled out and signed with the forms I have my new patients complete.

If your PDF document is a fillable PDF, then you may upload your original PDF and create a web form.
