Appointment field: Received submission with time slot of vacation date.

    i have a patient that submitted a form and picked a day in the schedule that i don't have on my schedule


    feb 19 is off my schedule

    how did the patient picked an offline date?

    I am sorry for the trouble caused to you. In which form you have received this submission? I did check your last edited form "Armitage Pharmacy Covid 19 Vaccination Questionnaire" and it seems to be disabled.

    Since you have HIPAA-compliant form, we are unable to check your form. I did clone your form, and it seems you have only enabled Wednesday between 02/15-03/01 with several vacation dates. Please check the screenshot below:

    Is this the form in which you have received submission for 02/19? I would suggest you to please enable this form, and we will take a look.

    I did activate the form

    i selected weekdays not Wednesday

    i removed all dates and i am still receiving appointments submitted from patients

    I have checked the most recent form in your account that has an Appointment field and submissions under it and it seems that the Appointment field is set to only accept appointments on the 19th of February and no dates were set as Vacation & Holidays.

    Could it be that your respondents were able to select February 19, 2021, in the Appointment field on your form because it is the only available date for them to select? Kindly confirm and feel free to let us know if you need any further assistance.
