I was suspended from my account because I tried my password too many times. Would like to reset.
Plus, my COVID liability waiver is not working on other people's ends.

Your account was suspended because you created a form to collect credit card sensitive information. The form was in the trash folder, so I've purged completely to avoid a future suspension.
