I sent a note yesterday. I can't get into a section in my forms area. I get a temporarily back end unavailable message. I need to download a patient form. Please advise asap.

We apologize for any inconvenience, actually we've been facing an issue with our HIPAA related back-end and some of our HIPAA accounts was affected, but our backend developers team has reported for its recovery, please try accessing the required form submissions from table view https://hipaa.jotform.com/tables/41545940736156, and try download it from there, please inform us if you still having this problem so we shall escalate it to our developers team again with your permission to perform a test submission on your form.
