Adding a fill in the blank question within a multiple choice field on a form

Good afternoon.

I am trying to add a question that would require a response within a yes and no field/question.

For example, there is a yes and no response, and if they respond is yes, then there is an additional box that needs to be filled out.

I am attaching a screencap for reference. I can't figure out how to do this without creating a new field directly below the original question, which is not what we are trying to accomplish.

I would like to apologize for any inconvenience. I'm afraid that showing or hiding an option in a field based on that field's or another field's answer is not possible.

If you want to collect the answer if they select "Yes" under the same field, you may consider using the "Other" option of the multiple-choice field.
