HIPAA form: unable to dowload submissions

    I have built a COVID-19 Vaccine intake & consent form because the pharmacy in which I manage are administering thousands of the COVID-19 Vaccine.

    I am receiving a lot of complaints because when patient's fill out the form, it doesn't bring them to page 2 where at the bottom of page 2, I have the submit form. I am needing this problem fixed where patient's fill out the 1st page and it automatically brings them to page 2.

    Also when we are trying to download the submissions into a pdf form, it isn't allowing us to download the attachments/pictures/uploads in which the patients submitted along with their form.

    This is extremely frustrating and I need help fixing these issues.

    I've checked your form and fount that it has 2 points that need to be adjusted:

    1- Page 2 is empty, you may deleted if not needed by clicking on "Page 2" then "Remove Page" label.


    2- The form has 2 Submit buttons, you need to keep only one to avoid any possible conflicts in the form.


    Regarding the PDF download issue, we've been reported of a temporary issue that affects the PDF downloads and attachments, this should be recovered by now, so please check again and let us know if it's still persistent.
