Customize PDF for download

If you customize one PDF to download, will it keep the customization for all future downloads?

I made edits to the online and PDF forms, yet when I download from the online form our patients fill out, it is not putting the signatures in the correct spots or the additional short answer questions I've added. They are being listed at the end.

Ideally, it should keep the same customization, but on the PDF Editor, you can also drag and drop the fields to the required spots too.
