Form freezes when a photo is attached

hi our customers are reporting when they get to the part of the form where they attach a photo; it freezes up. then they lose the whole form.

If so, I tried filling it and uploading a photo to it but the form did not freeze up.

I tested Piercing consent form but I was not able to replicate the issue. See gif below.

Clearing the form cache must have resolved the issue.
