Why is a frame displaying on embedded form?

Hello I am wondering why there seems to be frame around my forms when embedding on my website? Could I receive assistance removing the appearance of a frame, adjusting the code, or is the template I'm using the issue. My New Client Consultation Form does not appear in a frame, I would like if all my forms could be like that. I did reach out to my website host and was informed it's a form or coding issue
