having issues with form not converting properly to pdf

I have 3 forms that when I go to download the pdf responses, do not function properly and show an older version of that form. (current form was edited from an older form.) When you view the form, the form is current, but when you download the submission via pdf, it is all the old questions. The submission downloads to excel properly, but that is not my preferred format for this project. In this screenshot, this is the OLD information that appears when I download the pdf submissions now. The uploaded screenshot is the current form and what we should be seeing.

I think this PDF that I downloaded does resemble your latest design rather than the older version.
