Not able to retrieve form and cannot login

I am having difficulties logging in after creating a form. The form is now disabled and i am unable to retrieve it.

Great! The system just shut down while creating the form. I would love to understand which policy was violated so I dont do it again. I hope I dont have to re-create the form from scratch.
