Form not fully loading in Fillable PDF format

Dear JF Support Team,

I have made and published an online form. This form work properly, however when I attempt to upload to a Fillable PDF form, the form does not have any paragraphs or signature area.

My question is;

Do I have to make separate PDF forms for the same form ?
