Form not integrating with Hubspot

Hi there - we have a pilot of a form we've created and integrated into Hubspot that doesn't seem to be showing up in Hubspot. The form completes, but nothing shows up in Hubspot. I've done the integration for both and it seems to be setup properly - except for the results. The form is here: https://rjg.global/3u3DmZG.

I've tried to find it by searching and don't see it either.

We would like to apologize for any inconvenience. I have tested the issue you are reporting on a cloned version of your form but I was not able to reproduce it.

Thank you for your response. I’m completing the form and then searching for
the info that I put in the form to test. Perhaps it’s a Hubspot issue, or
easier / cleaner to just build it in Hubspot directly. Thank you for your
