Question About PDF uploads

    Hello!

    We currently use 123 Form Builder and are looking at a new integration for our Dog Daycare facility. We want to have a .pdf that they fill out on the website and then that .pdf gets emailed to us automatically when they check-in their dog. Is this possible on your platform. I've attached the .pdf we want to exactly use.

    Please advise.
