Dear JotForm
Now We established 21 tablets to draws patient evaluation for the hospital. Besides, we also use QR codes so that patients can evaluate from their private devices such as mobile phones, tablets, etc. I really want to know how many percentages of evaluation were made from patients' private deceives and our hospital's tablets. Does Jotform have that kind of report? Or what should I do to have that kind of report?

We don't have that kind of report. We recommend adding the "get user agent widget" and manually check the browser information from the widget. http://widgets.jotform.com/widget/get_user_agent

But in the Form Analytics, I saw that it has device section in which show the devices that respondents used to evaluate the forms
Is that analytic the way Jotform used to measure which devices responders used?
Besides when I use "Combine Submission Data" widget, I saw that it has "Submission Date/Time" and "Submission IP address".
and when I export it into CSV. file, it has "submission IP", "Submission Date/Time" and "Submission IP address" column like that
Can we based on "submission ID" and "Submission IP address" to trace back the devices that respondents used.

In the Jotform Analytics, we can see the device used by respondents.
Do you have any suggestions to improve the analytics?
As suggested before, we recommend adding the get_user_agent plugin.
When someone submit the form, you can see in the tables the kind of device they used.
Let us know if we can do anything else for you.

For Jotform Analytics, I could not export analytics into Excel file so I could not combine it into my form report. For example, from "My tables" I export an Excel file like this
Submission date Question 1 Question 2 Question 3
#### Yes No Yes
##### Yes Yes Yes
Now I want to make a report like this
Submission date Question 1 Question 2 Question 3 Submission Device
#### Yes No Yes Generic tablet
##### Yes Yes Yes Iphone 12
in which "submission Device" column comes from "Jotform Analytics"
For get_user_agent plugin, it just exports the "User Agent String" that mentioned about the user's browser or operational system. It does not mention the user's device, Tommasso. Our tablet used Android OS and patient's device OS might also use Android OS, so how could we differentiate between those devices.Best regards,

Unfortunately, we don't have any other way to get device information.
I'll report this as a missing feature to our team. Jotform reviews all feature requests we get and we spend a lot of time researching and working on new features, but at the moment, I don't have any details on if, or when, this will be released as we don't have a public road map. Whenever we do release new features, we'll announce them to all interested customers. We appreciate your interest and input! -
