Visitors having trouble scheduling Appt

Visitors are having issues setting up appointments. Says "incomplete values", tried it myself also came across the same problem. I was able to make an appointment from the Jotform admin side of things, but not from the outside?

I've cloned your form and made a test submission but was not able to replicate the issue.

I also could not find any issues on your Appointment field's setup, as such, I have instead cleared your form cache in hopes that it helps.

Kindly test it again. If the issue persists, please let us know the timeslot that you've selected and we would like to make a test submission directly on your form.
