Shipping calculations not adding properly

Hi,

I enabled shipping and configured the shipping costs, but they are not being added to the shipping cost below the subtotal.

Would you take a look at the form called Support Order Form and let me know how to fix?

I've tested out your form and the shipping costs calculates properly based on my tests. Perhaps, I've missed something? In that case, please elaborate on your inquiry further so we can further investigate.

Additionally, I saw that the first 2 products don't have shipping costs. Is this intentional?
