Forms disabled, tried to get in and changed password, but now suspended

Hi,

I hope you're well. On of our forms are giving this error: "Form not found. This form is disabled." And I'm guessing the other two are as well.

I tried to log in to see the problem, but password didn't work. Reset it, but it said it was suspended. Can you please help me to access the account and also figure out why the forms are unavailable?
