My account is suspended and needs to be reset

When I try to login it says login suspended and my forms is disabled. It wouldnt allow me to enter a ticket either. Somehow my phone is still logged in and submitting a ticket. On the regular computer I get an error message each time. Please reset my account so I can continue to use it and send out my form.
