Form has been blacked out

Hi there,

When we put in the password to https://www.dublincityfm.ie/programmeinfo/ we get this blacked out screen:

I've tried pressing the "X" in the top right corner but it doesn't respond. I've checked our account and we have not reached our limit. Can you help please? Thank you!

How have you embedded the form on your website?

Could you embed using the iFrame code and then check; https://www.jotform.com/help/148-getting-the-form-iframe-code/?
