How can I add a customized shipping form to my order form?

We are running a catering and have set up our online menu. However, we have different delivery fees based on different locations in town. How do I add the option of the client choosing their desired delivery location and the set delivery fee automatically gets added to the total before checking out?

Unfortunately, the shipping option cannot be changed based on location.

You may consider adding a product called shipping for the customer to select if they would like shipping and to add each location with a different fee.
