File upload fails on the form template

I fill up the form and upload the image and then submit. Then the warning red pop up written as 'file upload failed, please delete and upload again'.

You need to fill out the live version of the Jotform form. May I ask where you got the form from? Did someone provide you with that URL? Or did you just find it on our form templates?
