The response form that I received via email worked fine except the signature image didn't come through. Help. Thanks!

The possible reason that there's no sign on the form is the user skipped it.
I've cloned your form and tried it myself and I receive the form with my signature on it.
In order to avoid this thing happening again make the signature required*
