I created a survey, and now all the fields are gone - did I not save?

i've done my first survey and i do not have it at my profile in my surveys :( is it possible, it did not saved?

can i somehow restore access to this survey?

I see that your changes have not all been lost:

Select the form revision history and restore the changes that you lost.
