Help! Need form with two columns

Hi there,

I would like to position a box with text on the right and have my entry fields on the left. 

I have used the shrink tool to do this but this pushes the next fields to the bottom and I want them to display underneath.

Also I would love to create a slight background (maybe #b3c8e8) for the information text box titled "What's included in the prize?" Also is there anyway to create rounded corners on the background colour?
