Jotform Widget for Sidebar

I installed the plug-in that is supposed to have a Jotform widget for sidebars, but when I try to activate it, I get a message saying "This plug-in does not have a valid Header" and it fails to activate.  Please help!

I believe you are referring to the JotForm plugin for Wordpress, is that correct?

Please make sure to follow these installation steps:

 

Upload jotform-integration.php to the /wp-content/plugins/ directory Activate the plugin through the 'Plugins' menu in WordPress Place the [jotform id=XXXXXXXXXXXXXX] short code in your pages or posts. Replace the XXXXXXXXXXXXXX with your form's ID.

If the issue persists please Re-Download the plug-in from:

http://wordpress.org/plugins/jotform-integration/ and try again.
