We are locked out of Jotform and our customers can not fill out any forms.

    As of this morning we are unable to log in to Jotform. Also none of our customers can use the forms we had set-up through Jotform. When you try to access the Jotform forms from our companies website it says the form is disabled. So basically none of our customers have been able to fill out a form and get service from us because of the forms being disabled. So basically been losing business all day.

    Hi there,

    I just got an update from our Review team. Here's what they said.

    Gathering this type of data violates our Terms of Use.

    Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar to "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.

    They also noticed you are using copyright logos: to reactivate your account, kindly provide us the following information:

    1. The URL where your form is used.
    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #3576619 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
