My form doesn't show the fields when clicked 'Yes'

I added some conditions under which some fields are or are not shown. This does seem to work for the option 'no' in my form, but doesn't work when clicked 'yes'. Do you recommend to get rid of the conditions or did I do something wrong in building the form?

It's quite important that the form is working correctly, I hope you can answer my question soon.

(The form I'm talking about is shown on http://gullivermagic.com/contact.html. When clicked 'yes' at the question 'Wilt u direct een vrijblijvende reservering doen?' some fields should appear, similar to when clicked 'no'.)
