    To Whom it May Concern:

We have published a scholarship application through JOT FORM. When completing the survey, the two personal statements we ask are, What is one piece of advice you would give incoming freshman? and What is the most difficult challenge you have faced and how did you handle it?

But when we DL/print the submission, this is what the questions say: How are the current events of the world affecting you or your views of the future? and What have you learned from someone who is very different from you?

    PLEASE HELP

You can update those labels manually or recreate the fields in your PDF document in the PDF Editor.

I would suggest you recreate them by deleting the fields with incorrect labels and then add them again from the Form Fields. This should give you an updated field label. The answers will not be affected.

    Here's how:

    1. Load your form in the PDF Editor or click this link https://www.jotform.com/pdf-editor/10210905300221032

    2. In the PDF Document, delete the two fields you mentioned with incorrect labels. After that, re-add these fields from the Form Fields tab. Example:

After that, download your PDF again and you should correct the updated labels.
