I can't get in to my forms, can't log in to account. Keeps going back to sign up page.

    When I try to go to the link for one of my forms, I get the message that the form is not found and form is disabled.

    I can't sign in at all.

    I have no idea what you are talking about. There are no fake service
    offers on the forms. They are release forms to release medical records and
    initial intake forms for new patients. I think you have made a mistake.
    If not, please be more specific about what fields on the forms are a
    problem. This is interfering with a medical practice.

    1. The URL where your form is being used/embedded.

    2. Your website or any proof of affiliation to a legitimate institution.

    3. Share with us documents that will prove that you are authorized to offer such services.
