When I try to go to the link for one of my forms, I get the message that the form is not found and form is disabled.
I can't sign in at all.

offers on the forms. They are release forms to release medical records and
initial intake forms for new patients. I think you have made a mistake.
If not, please be more specific about what fields on the forms are a
problem. This is interfering with a medical practice.

2. Your website or any proof of affiliation to a legitimate institution.
