Inauthentic job applications and unauthorized use of personal information

So, I was recently extended a job opportunity and applied via Jotforms. The person who posted the listing is now using my name and address to apply to more fraudulent positions. I am concerned as I have received fraudulent checks and when I looked up the company, I found they were listing companies in the names of their victims.

I have checked emails logs and located the forms you have filled. All accounts related to them are suspended, so the user doesn't have access to any data they have collected.
