Email notices of submissions are displaying "Type a Question"

Email notices of submission are displaying "Type a Question" on my BSAFE Registration contract instead of the field questions and headings that it should display? It actually DOES include the questions in the first part of the form submission but not the middle and button section. What can I do to fix this?

    Hi there,

I did check your form and in the email notification template, I see you have questions there that are labeled as "Type a Question'.

I would suggest you edit the email notification template, and update the text according to your preferred label.

I hope this helps. Let us know if you need any further assistance.

Thank you for your response. I can certainly do that and will now I guess. I use several Jotforms for clients and I just checked and ALL of them pull out the questions/text automatically....as does this one when I pull a PDF of the form submission Why isn't this form pulling out the information like it has in the past? And actually should. Perhaps there is a larger issue than just this happening with my form.

Oh and why would it pull it half of the text entries correctly in the form, and not the other half? Just questions to consider at your end. I'll update the template at my end as you suggested.
