The File Upload shows a "Failed" error message while trying upload a file

i insert the FIle upload widget, ensure all the settings are correct, bumped up the file size from 1MB to allow camera phones to upload pictures, when running the form on the web uploading the picture fails... failes from phone and computer upload.. any ideas ? please help thanks in advance
