Response doesn't indicate date selected

    Hello,

    I have a form where people sign up to pick up food for the homeless. I've used a widget because we only need so many people per sign up date. Unfortunately, when our lead volunteer receives the notification email (screen shot attached), the email doesn't indicate the date the volunteer selected, just their name and telephone number. Is there a way she can have the date they selected on her notification email? What am I doing wrong?

    Hello,

    It appears you are still using the old Autoresponder theme. Could you kindly create a new Autoresponder theme and delete the old one? It should fix the issue.

    Let us know if the issue still persists.
