I have the sent the form from the website and an app I created however this all thats being recieved.

The field tag that is supposed to be replaced with the actual data entered by the user when the notification is sent to email is not on your form notification. You must add it there so it will appear on email.

This worked for this form. However on my incident report form the email address is not working I have set up any suggestions?

Did you follow the same steps and check if your field tags are correct?
I think the easiest way to resolve this is to recreate your existing form notification so the tags are re-generated with the correct one. Just delete your existing form notification and create a new one.
