Unable to open my form in smartphone.

I have been trying to access my form on smartphones but it seems the site is froze, my form will not download on various phones, is there a server problem?

Are you trying to access your form with its direct URL or have you embedded it in your web page?

I did check your form in my iPhone device and it seems to load without any issues. Please be noted that our forms are already optimized for mobile phones. The issue comes up when you embed your form in your web page. Please share the web page URL and we will take a look. You may also like to embed your form using its iFrame embed code and see if that solves your problem:  http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
