Urgent Form Issue

Hi Folks,

We have had two issues with our form that is not allowing it to be used.

1. The captcha was the first issue as it would not load each time, so we hadto remove it.

2. Now when the user hits the Submit button, it get's stuck and does not process

The standalone form works here on my end, so I want to know if your form is embedded on a webpage.
