cant login the account

i just created my form an hour ago , and now when i tried to login the account again , the website said it was suspended!

i just sent others the url of the form ,and i cant get their reply now!!!

Your account was automatically suspended by our phishing filter.  I believe it mistook your form for a login phishing form as it follows the same format as most of them do.  I checked into your form and found this is not the case.  I activated your account and whitelisted your form.  You should no longer have any issues.  Please let us know if there is anything else we can assist you with and we will be happy to help.
