I've already searched for a way to include custom Javascript to detect the presence of Flash Player (and major version) in a form, but it appears that I cannot because the form is hosted on the JotForm site. I did find a widget which collects browser, OS, language, resolution, timezone and user agent, but not Flash information.

Please let me know if there is a widget that does this, or if there is a way to enable this somehow.

    Thank you so much.

Have you tried this widget? You can embed Flash application right in your form with it.

I haven't tested yet, but it should detect if the browser go no flash.
