How do I take a survey off of the end of a form?

There is a satisfaction survey that was set to come up after the form is submitted and when I cloned it the same survey comes up and I want to delete the survey.

Can you please tell us which of your form is this?

I checked your forms, and this one http://www.jotformpro.com/form/40306400442943 seems to be the recently used. But I could not find or see the "survey" you mentioned about.

Please give us more details and we will check.
