    Hi Scott,

I look likes the custom Autoresponder Email in your form had been corrupted. This could be the reason for some broken html codes to appear in the message template.

You can fix this by creating a new Autoresponder Email

    User Guide: https://www.jotform.com/help/26-Creating-a-Form-Autoresponder

And make sure when you customize the email template and convert it to Plain text mode, you only add/paste text-only data into the message.

Do Not paste copied text code for other word editors such as MS Word(or similar) because it will cause problem with the formatting. Since your custom email message is plain text only, then use also plain text data only.

Hope this help. Inform us if issue remains.
