((form is disable)) all my forms disable i need help

i create a form and then show message ((form is disable)) in all my forms any one help me?

and i create downloader forms

like windows downloader example : if he select ((windows 7 ultimate 32 bit)) when he press the submit button he will redirect to download url and my forms like this

minecraft maps downloads

minecraft plugins downloads

and other

i dont do any thing wrong
