Is there a way to ask a question and have a yes or no response in the box?

    I couldn't find a form tool where it gives you the box to write a sentence/question then have the response with it. I did a text box and then put a button response below; but when you get a submission, it only shows the response without the question.

    I don't think that will work because this is the question:

    We also give permission for any "Youth Group" picture that includes my/our child(ren) to be used on Grace Baptist Church’s website and facebook page---

    Then I want a yes and no response for them to choose.

    Can you confirm that what you wanted to happen is like this.. in the submission data you want the answer to be like this.

    From what I understand so far, what you wanted to happen is to have the Question also included in the Answer.

     

    Please clarify further so we can look for an appropriate solution.

    Yes, what you have is what I want (I think). With that, how do they answer so we know what they desire and it will show up on their   submission?
