The Limo Form [Errors and conditions are not saved]

Hi JotForm,

Its a very beautiful program that you guys created but i do have some small issues.

I am trying to work on the Limo Form to be like the following

1- (look at the pickup and Drop off) I used the conditions to be able to change from Airport to Hotel to street Address to As directed but part of the conditions are never saved!

2- I always am getting errors when i go to payments! 

3- I am not able to find out how (when i select one type of service) to make the other types of service automatically not selected! The same issue with preferred Vehicle!

4- I tried to change the height for the preferred vehicle but sometimes works fine and other times a huge space between the pictures and the following fields!

My first mistake was that i made a copy from the types of service to save time but i guess "not sure yet" that when i work on one the other copy get missed up!

The second i should have used the Radio button instead of check box.

I will try them and see despite the fact i am still not aware of the real solution

I tried to correct some of the issues and I could resolve the selection issue by switching to Radio Button but the Conditions are still sort of the same. It works with some and does not work with the other

I did check the conditions you have added in your JotForm and found that there are several duplicate conditions. Please check the screenshot below:

You should actually merge all these duplicate conditions and that should solve your problem. Please be noted that if you want to show / hide multiple form fields with one single conditions, there is a better way to organize your conditions. You should move all the form field which you want to show in a condition to a "Form Collapse" field. Now the condition you can just add one action to show / hide the form collapse which will in turn show / hide all the fields.
