I wish to upload into my form a data file for User Response

    The data file can be either Word or Excel - whichever suits.

    The existing data file looks like this:

    (ie, 3 columns).

    TITLE

    COMPOSER

    The two left columns are fixed, and the user fills in the right hand column.

    There are 67 rows in the data file.

    You can instead use the Spreadsheet Widget to accomplish this.

    The Spreadsheet widget will allow you to have a fill-able Excel like data that the form respondent can use.
