Thanks for reaching out to Jotform Support. It appears that you are referring to a form you submitted. Please reach out to the form owner so you can direct your concern to them. If you have received a confirmation email after submitting the form, you can use the Reply-to option on that email to contact the form owner and raise your concern to them.

If you are unfamiliar with Jotform, Jotform is used to create online forms. We have a lot of users using Jotform to create order forms, survey forms, quiz forms, and more.
