I have ongoing problems with customers that are leaving my site due to your Security check system.
I even am sending my customers to fill out a certain form, where it resides on your server (not embedded on my website thus) on http://www.jotform.com/Giftsngadgets/customer_survey
And the still get the below security bullshit screen.
If you do not remove this whole ridiculous system before the end of the month, I will take my business elsewhere.

The security check is determined by CloudFlare, not by Jotform. All you need to do is make sure to pass the captcha in order to lift the security check. It's IP based, based on the information from CloudFlare.
You seem to have posted this issue a few days ago: http://www.jotform.com/answers/442635. I think it's time for you to tell your ISP about the problem. I have a doubt that its ISP related thing.
