How to do a purchase order form Multiple Variant With Differential/Special Prices

Hi,

Do you have a tutorial, or could you please explain step by step how to do a multiple variant with different special prices purchase order form.

There are some discounts I'd like to offer on my form whose values I want to show as different prices. I came across the snap shot of the special prices in the forum and it's ideal for what I want to do, however I've been trying to create a purchase form the same and after many attempts are no closer to success.

The payment form is a paypal pro form.
